Emergency Contact Form – Must be completed for all Foothill Instrumental Music students that travel

Individual Donation Form and Charms Information – Students submitting for Individual Donation for all Ensembles.  All Students should complete Charms Information Update

Volunteer Interest Form – Please fill out this form if you are interested in volunteering.

Annual Volunteer Clearance Form – This for Parents Interested in Volunteering

Band Interest Form – Student Information Sheet. Only required once a year to submit interest.

Marching Band Uniform Size Questionnaire – Required for all students in Marching Band

Reimbursement Form – Should be used if you need to be reimbursed for a purchase you have made on behalf of FHS Band. Once completed, please email to  treasurer.general@foothillmisic.org or  treasurer.student@foothillmisic.org

Trophy Sponsorship Form – Interested in donating or sponsoring in the Foothill Band Review? Please complete and email this form to president@foothillmusic.org
